1) Passport photo – 1no.
2) Legal stamp – 3nos
3) Nu.300/- for drug test.
4) Identity card photo copy – 1
5) Valid medical certificate
6) NOC from working agency if employed
7) Photocopy of academic marksheet and ECCD training certificate
